Neuro-Oncology Department I
The First Department of Neurosurgery of the First Affiliated Hospital of Dalian Medical University has 35 beds. It is mainly divided into the trauma group, neurovascular intervention group, cerebrovascular disease group, and neuroendoscopic treatment group. It admits more than 600 inpatients annually, performs more than 500 operations annually, and has an annual bed-utilization rate of >99%; the conformity rate between admission and discharge diagnoses is >98%.
Technical features and development prospects of the main development directions: The Dalian area has a high incidence of cerebral aneurysms and hypertensive intracerebral hemorrhage. As early as 1988, our department began performing direct clipping surgery for intracranial aneurysms. With the clinical application of DSA, microscopes, and computers, the department subsequently introduced endovascular embolization treatment for intracranial aneurysms, microsurgical aneurysm clipping, and eyebrow-arch keyhole clipping of intracranial aneurysms. The annual number of aneurysm operations exceeds 300. The diagnosis and treatment of subarachnoid hemorrhage and intracranial aneurysms is one of the distinctive features of our department and is also a key research direction of the discipline. The diagnosis and treatment of various complex intracranial aneurysms not only ranks among the domestic leaders in treatment volume, but also ranks among the domestic leaders in the management of complex intracranial aneurysms. At present, treatment of cerebrovascular diseases accounts for approximately 80% of the total in southern Liaoning. In 2012, the Dalian Cerebrovascular Disease Treatment Center and Hemorrhagic Cerebrovascular Disease Treatment Base were successfully established.
In addition, our department has ranked among the leading departments in Dalian for decades in both the volume and difficulty of brain-tumor surgery. It is also at the forefront in the development of new technologies, such as minimally invasive surgery for brain tumors.
Technical advantages: The technical advantages currently possessed by this discipline among comparable disciplines and bases in China: Our department began surgical treatment of intracranial aneurysms in the late 1980s and now has more than 20 years of clinical treatment experience. In the treatment of this disease, our department was historically among the first in Liaoning and even Northeast China to complete a number of pioneering procedures, including unilateral craniotomy with bilateral internal carotid artery aneurysm clipping, basilar tip aneurysm clipping, endovascular embolization of multiple intracranial aneurysms, reconstructive clipping of giant intracranial aneurysms, stent-assisted remodeling with a covered stent for fusiform aneurysms, and CT-guided neuroendoscope-assisted minimally invasive surgery for hypertensive intracerebral hemorrhage. It established a leading advantage in the region at an early stage. Professors Xu Yinghui and Liu Rongyao of this base are at the domestic leading level in the surgical management, diagnosis, and treatment of complex aneurysms. They were the first in China to perform eyebrow-arch keyhole clipping of intracranial aneurysms, shortening operating time, reducing the burden on patients, and improving surgical efficacy. This base has now established a scientific and rational diagnosis-and-treatment system for hemorrhagic cerebrovascular diseases. Patients are reasonably selected and grouped through pre-assessment, while the hospital’s green emergency treatment channel for cerebrovascular diseases provides convenient conditions for implementing various clinical technologies. To date, our department has completed treatment for more than 2,000 intracranial aneurysm cases, of which more than 40% were referred as complex cases from surrounding areas. In addition, our department undertakes more than 100 consultations each year for tertiary or higher-level hospitals related to this technical procedure. Over the past three years, this base has completed approximately 200 such procedures per year on average, with a cure-and-improvement rate of over 95%; the rates of postoperative complications and mortality are below 3%, reaching an internationally advanced level.
