The Department of Ultrasound currently has 48 professional and technical staff, including 36 physicians and 4 full-time nurses. The department is divided into 4 subspecialty groups: musculoskeletal comprehensive, obstetrics and gynecology, cardiac, and interventional, and comprehensively provides all routine ultrasound and interventional ultrasound minimally invasive diagnostic and therapeutic services. “Integrated Ultrasound Visualization-Based Precision Diagnosis and Treatment of Shoulder Joint Lesions” is a clinical specialty technology project in Guangzhou. The Department of Ultrasound of the Second People's Hospital of Guangdong Province was among the earliest in South China to comprehensively provide adult and pediatric musculoskeletal ultrasound, and the Institute of Musculoskeletal and Sports Medicine Ultrasound was officially established in 2019. It has distinctive technical strengths in the integration of ultrasound diagnosis and treatment of the musculoskeletal and sports systems, key technologies for contrast-enhanced ultrasound in female infertility, research and development of artificial intelligence ultrasound-assisted systems, precision diagnosis and treatment through molecular imaging, and the integration of minimally invasive diagnosis and treatment of thyroid nodules, among other areas; its integrated ultrasound diagnosis and treatment technology for the musculoskeletal and sports systems is at the leading level in South China. Related research has been conducted with support from national- and provincial/ministerial-level projects and published in top SCI journals such as Radiology, and has received the Second Prize of the Guangdong Provincial Science and Technology Progress Award and other scientific research achievement awards. Based on a 5G full-scenario smart hospital, the Department of Ultrasound of the Second People's Hospital of Guangdong Province took the lead in China in establishing a multilevel regional medical consortium remote intelligent ultrasound platform with Zhaoqing Central People's Hospital, Sihui Second People's Hospital, and other medical consortium institutions. With the support of 5G, the multilevel regional medical consortium remote intelligent imaging platform leverages the advantages of immediate and efficient data transmission to realize smart healthcare application scenarios such as remote diagnosis, teaching and training, and academic discussion. It breaks down the temporal and spatial barriers caused by geographical factors and, through “provincial-city-county-town-village” multilevel coordination, has served as a good model for addressing the difficulty of bringing medical resources down to primary healthcare institutions in terms of personnel, technology, and other aspects, while also providing new exploratory ideas for challenges such as difficulty accessing medical care and the uneven distribution of medical resources. Introduction to the Specialty Group Teams of the Department of Ultrasound Examination Project Introduction Comprehensive Group Photo Introduction to the Comprehensive Group's Ultrasound Diagnostic and Therapeutic Projects: 1. Ultrasound examination of abdominal organs: the digestive system, urinary system, abdominopelvic region, etc.; 2. Ultrasound examination of superficial organs and superficial masses: the thyroid, breasts, salivary glands, eyeballs, lymph nodes, andrology, superficial tissue masses, etc.; 3. Ultrasound examination of the musculoskeletal and sports systems: this is a signature diagnostic and therapeutic project of the Comprehensive Group. It was initiated relatively early in Guangdong Province and has considerable influence in the field. The group currently comprehensively performs ultrasound examinations of the six major joints and the musculoskeletal system throughout the body, as well as ultrasound-guided musculoskeletal interventional procedures, and has trained more than 30 person-times of musculoskeletal visiting trainees. It has served as chief editor and chief translator of 3 musculoskeletal books, participated in compiling 4 books related to musculoskeletal medicine, and led 8 musculoskeletal-related projects (including 2 National Natural Science Foundation of China projects and 5 provincial/municipal-level projects), and in recent years has published nearly 20 musculoskeletal-related papers. The Institute of Musculoskeletal and Sports Medicine Ultrasound was established, with Chief Physician Liu Hongmei serving as its director. It has hosted 5 South China Musculoskeletal Ultrasound Summits and undertaken 1 premium course of the Ultrasound Physicians Branch of the Chinese Medical Doctor Association—the Musculoskeletal Ultrasound “Spark Program.” In recent years, it has focused on primary-level training and promotion of musculoskeletal ultrasound diagnosis and treatment technology.; 4. Ultrasound examination of abdominal and peripheral vessels: ultrasound examination of the renal vessels, hepatic vessels, abdominal vessels, and vessels of the neck and extremities; 5. Contrast-enhanced ultrasound and elastography: contrast-enhanced ultrasound examinations of abdominal organs, superficial organs, fallopian tubes, peripheral vessels, joint cavities, and bursae have been performed, and elastography examinations of the liver, thyroid, breasts, and muscle tissue have been performed. Interventional Group Photo Interventional Ultrasound Diagnostic and Therapeutic Projects and Projects Conducted: 1. Ultrasound-guided organ/tumor needle biopsy: the liver, kidneys, lungs, pleura, thyroid, breasts, lymph nodes, etc. 2. Ultrasound-guided catheter drainage of abscesses/effusions: percutaneous transhepatic biliary drainage/percutaneous transhepatic gallbladder drainage (PTCD/PTGD), catheter drainage of liver abscesses, moderate-to-severe acute pancreatitis and pancreatic pseudocysts, CVC, pleural, abdominal, and pericardial effusions, etc. 3. Ultrasound-guided sclerotherapy for cysts/superficial varices: hepatic and renal cysts, thyroid cysts, branchial cleft cysts, lymphatic cysts, ovarian cysts, lower-extremity superficial varices, etc. 4. Ultrasound-guided minimally invasive ablation treatment: microwave/radiofrequency ablation treatment of benign thyroid and breast nodules, small liver cancer, liver metastases, etc. Advantages of ultrasound-guided radiofrequency/microwave ablation: real-time ultrasound-guided evaluation, safe and reliable; precise ablation using a needle instead of a scalpel, minimal trauma, rapid recovery, and no scarring. Principle of radiofrequency/microwave ablation: Using cold-circulation radiofrequency/microwave technology, under real-time color Doppler ultrasound guidance, the location of the treatment point is determined, and the power, time, and range of the radiofrequency/microwave ablation needle are precisely controlled by the control system. Without damaging normal tissue, the diseased tissue is made to produce a biological hyperthermic effect, causing nodule and cyst tissue to dehydrate, coagulate, undergo apoptosis, and become necrotic during thermal coagulation, and finally be naturally absorbed or excreted by the body, thereby achieving the goal of curing thyroid/breast nodules and liver tumors. Local anesthesia is used throughout the treatment. Appointment process for interventional ultrasound diagnosis and treatment: Nodule discovered during a previous examination or physical examination----evaluation in the ultrasound interventional examination room---completion of preoperative examinations---appointment of surgery time---15-minute postoperative observation---may leave only if there is no discomfort. Cardiac Group Photo Department of Ultrasound Cardiac Examination Projects: routine transthoracic echocardiography, transesophageal echocardiography (transesophageal three-dimensional echocardiography), left-heart contrast echocardiography, right-heart contrast echocardiography, intraoperative monitoring during cardiac surgery and interventional treatment, and bedside cardiac ultrasound examination for critically ill patients. Left-heart contrast echocardiography: a new technology that has only been applied clinically in recent years, mainly divided into myocardial contrast echocardiography (MCE) and cardiac chamber opacification (LVO). MCE can be used to evaluate myocardial microcirculatory perfusion and accurately determine the extent and severity of coronary microcirculatory disorders and acute and chronic ischemic heart disease; the main clinical indications for LVO are: 1. Quantitative evaluation of left ventricular volume and ejection fraction; 2. left ventricular apical hypertrophy, thrombus, and aneurysm; 3. left ventricular myocardial noncompaction; 4. differentiation of intracardiac masses. Right-heart contrast echocardiography: the main indications are: 1. patients with cryptogenic stroke, migraine, etc. who are suspected of having a patent foramen ovale; 2. patients suspected of having a pulmonary arteriovenous fistula; 3. patients suspected of having anomalous venous drainage (such as a persistent left superior vena cava draining into the left atrium, etc.); 4. patients with decompression sickness, such as divers or pilots; 5. assessment of the closure effect after patent foramen ovale closure. Transesophageal echocardiography: the probe is inserted directly into the esophagus to examine the cardiac structures from behind the heart, effectively avoiding interference with ultrasound waves from the chest wall, pulmonary gas, lung tissue, etc., and producing clearer images. It has obvious advantages in detecting left atrial appendage thrombus and patent foramen ovale and facilitates ultrasound monitoring and evaluation during cardiac surgery. Obstetrics and Gynecology Group Photo Introduction to Prenatal Ultrasound Examination Projects and Examination Notes: 11-13+6 weeks of gestation ultrasound examination (NT) 11-13+6 weeks of gestation ultrasound examination is one of the examinations that includes NT measurement; NT is one of the screening indicators for Down syndrome in the fetus. Because of factors such as fetal position and the pregnant woman's abdominal wall, some fetuses may not have a standard plane obtained for NT measurement and therefore cannot be included in the combined risk assessment system for fetal Down syndrome in early pregnancy. I-level prenatal ultrasound examination I-level prenatal ultrasound examination is mainly used to determine the number of fetuses and fetal presentation and to measure fetal growth parameters, amniotic fluid, and placenta; it is not primarily intended to screen for fetal malformations. Ⅱ-level prenatal ultrasound examination II-level prenatal ultrasound examination is mainly used to screen for fetal anencephaly, severe encephalocele, severe open spina bifida, severe thoracoabdominal wall defects and visceral eversion, single-chamber heart, and lethal skeletal dysplasia. Because of the influence of multiple factors (the pregnant woman's abdominal wall, fetal position, amniotic fluid volume, gestational age, anatomical structural abnormalities not yet becoming visible due to disease progression, etc.), the diagnostic accuracy and detection rate for mild cleft lip, cleft palate, congenital heart disease, and skeletal system malformations are low. No abnormality seen in the fetal four-chamber heart cannot rule out all congenital heart diseases; problems related to the fetal hands, feet, ears, sex, and genitalia are outside the scope of ultrasound examination. Please understand that ultrasound examination has limitations, that changes during fetal development are uncertain, and that follow-up is important. III-level prenatal ultrasound examination (three-dimensional, four-dimensional) III-level prenatal ultrasound examination is mainly used to diagnose fetal anencephaly, severe encephalocele, severe open spina bifida, severe thoracoabdominal wall defects and visceral eversion, single-chamber heart, and lethal skeletal dysplasia. No abnormality seen in the fetal four-chamber heart cannot rule out all congenital heart diseases; ultrasound displays the long bones of the fetal limbs and does not include examination of the hands and feet, so it cannot rule out abnormalities of the fetal hands and feet; problems related to the fetal ears, sex, and genitalia are outside the scope of ultrasound examination. Please understand that ultrasound examination has limitations, that changes during fetal development are uncertain, and that follow-up is important. General Knowledge of Fetal Ultrasound Examination 1. Does ultrasound affect the fetus? The ultrasound output energy used in obstetrics is very low, and a single area is not scanned continuously for a long period. The dose of ultrasound examination has no cumulative effect, so even repeated examinations will not cause harm to the fetus or mother. To date, there have been no reports of embryonic death or fetal malformation caused by ultrasound examination. 2. How many ultrasound examinations should be performed during pregnancy? The current general view in international medicine is that at least 4—5 ultrasound examinations should be performed throughout pregnancy. The purposes and significance of ultrasound examinations at different periods and different gestational ages are not exactly the same, and they cannot replace one another. Fetal growth and development are a dynamic process, and different fetal malformations can appear at different stages of development. 3. When should a pelvic floor function examination be performed? Women with pelvic floor dysfunction may develop pelvic organ prolapse, urinary incontinence, fecal incontinence, sexual dysfunction, and chronic pelvic pain. Pregnancy and childbirth are major predisposing factors for PFD. The period from 42 days postpartum to 1 year is the best time to repair the pelvic floor muscles, and the first 6 months postpartum is the golden period for repair. After childbirth, please undergo pelvic floor ultrasound examination as soon as possible, have pelvic floor function assessed, and receive rehabilitation training in a timely manner to avoid missing the optimal period for pelvic floor rehabilitation.
