The Department of Ultrasound of Guangdong Second Provincial General Hospital was among the earliest in South China to comprehensively carry out adult and pediatric musculoskeletal ultrasound, and the Institute of Musculoskeletal Sports Medicine Ultrasound was officially established in 2019. It has distinctive technical strengths in integrated ultrasound diagnosis and treatment of the musculoskeletal system, key technologies for contrast-enhanced ultrasound in female infertility, development of artificial-intelligence ultrasound-assisted systems, precision diagnosis and treatment using molecular imaging, and integrated minimally invasive diagnosis and treatment of thyroid nodules, among which integrated ultrasound diagnosis and treatment of the musculoskeletal system is at a leading level in South China. Relevant research has been conducted with support from national- and provincial/ministerial-level projects and published in top SCI journals such as Radiology, and has received research achievement awards including the Second Prize of the Guangdong Science and Technology Progress Award. Based on a 5G all-scenario smart hospital, the Department of Ultrasound of Guangdong Second Provincial General Hospital was the first in China to establish a multi-level regional medical consortium remote ultrasound smart platform with Zhaoqing City Central People’s Hospital, Sihui Second People’s Hospital, and other medical consortium member institutions. With 5G support, the multi-level regional medical consortium remote imaging smart platform leverages the advantages of immediate and efficient data transmission to realize smart healthcare application scenarios including remote diagnosis, teaching and training, and academic discussion. It breaks through the temporal and spatial barriers caused by geographical factors and, through multi-level coordination across “province-city-county-town-village,” has played a good demonstrative role in addressing the difficulty of bringing medical resources down to primary-level medical institutions in terms of personnel and technology, while also providing new avenues for exploring solutions to challenges such as difficulty accessing medical care and the uneven distribution of medical resources. Department of Ultrasound Subspecialty Team Introduction Examination Items Introduction Comprehensive Group Composite Photo Comprehensive Group Ultrasound Diagnosis and Treatment Items Introduction:1. Abdominal organ ultrasound examination: digestive system, urinary system, abdominopelvic cavity, etc.;2. Ultrasound examination of superficial organs and superficial masses: thyroid, breasts, salivary glands, eyeballs, lymph nodes, andrology, superficial soft-tissue masses, etc.;3. Ultrasound examination of the musculoskeletal system: this is a distinctive diagnosis and treatment service of the Comprehensive Group, which was initiated relatively early in Guangdong Province and has substantial influence in the field. The group now comprehensively performs ultrasound examinations of the six major joints and the muscles and bones throughout the body, as well as ultrasound-guided musculoskeletal intervention procedures, and has trained more than 30 musculoskeletal visiting trainees. It has served as chief editor and chief translator of 3 musculoskeletal books, participated in compiling 4 musculoskeletal-related books, led 8 musculoskeletal-related projects (including 2 National Natural Science Foundation projects and 5 provincial/municipal-level projects), and published nearly 20 musculoskeletal-related papers in recent years. The Institute of Musculoskeletal Sports Medicine Ultrasound was established, with Chief Physician Liu Hongmei serving as its director. It has hosted five South China Musculoskeletal Ultrasound Summit Forums and undertaken one edition of the Chinese Medical Doctor Association Ultrasound Physicians Branch featured course—Musculoskeletal Ultrasound “Spark Program”. In recent years, it has focused on grassroots training and promotion of musculoskeletal ultrasound diagnosis and treatment technologies.;4. Ultrasound examination of abdominal and peripheral blood vessels: ultrasound examination of renal vessels, hepatic vessels, abdominal vessels, and vessels of the neck and extremities;5. Contrast-enhanced ultrasound and elastography: contrast-enhanced ultrasound examinations of abdominal organs, superficial organs, fallopian tubes, peripheral vessels, joint cavities, and bursae have been carried out, as have elastography examinations of the liver, thyroid, breasts, and muscle tissue. Interventional Group Composite Photo Interventional Ultrasound Diagnosis and Treatment Items and Procedures Offered:1.Ultrasound-guided organ/tumor needle biopsy: liver, kidney, lung, pleura, thyroid, breast, lymph nodes, etc.2.Ultrasound-guided catheter drainage of abscesses/effusions: percutaneous transhepatic biliary/gallbladder catheter drainage (PTCD/PTGD), catheter drainage for liver abscess, moderate-to-severe acute pancreatitis and pancreatic pseudocysts, CVC, pleural, abdominal, and pericardial effusions, etc.3.Ultrasound-guided sclerotherapy for cysts/superficial varicose veins: hepatic and renal cysts, thyroid cysts, branchial cleft cysts, lymphatic cysts, ovarian cysts, lower-extremity superficial varicose veins, etc.4.Ultrasound-guided minimally invasive ablation treatment: microwave/radiofrequency ablation treatment of benign thyroid and breast nodules, small hepatocellular carcinomas/liver metastases, etc. Advantages of ultrasound-guided radiofrequency/microwave ablation: real-time ultrasound-guided assessment, safe and reliable;precise ablation using a needle instead of a knife, minimal trauma, rapid recovery, and no scar. Principle of radiofrequency/microwave ablation: Using cold-circulation radiofrequency/microwave technology, under real-time color Doppler ultrasound guidance, the location of the treatment point is determined, and the control system precisely controls the power, time, and range of the radiofrequency/microwave ablation needle. Without damaging normal tissue, the lesion tissue is made to produce a biological hyperthermic effect, causing nodule and cyst tissue to dehydrate, coagulate, undergo apoptosis, and become necrotic during thermal coagulation, and ultimately be naturally absorbed or excreted by the body, thereby achieving the purpose of curing thyroid/breast nodules and liver tumors. Local anesthesia is used throughout the treatment. Interventional ultrasound diagnosis and treatment appointment process: nodule discovered during a previous examination or physical examination----assessment in the ultrasound interventional examination room---completion of preoperative examinations---appointment of surgery time---15-minute postoperative observation---may leave if there is no discomfort. Cardiac Group Composite Photo Department of Ultrasound Cardiac Examination Items: routine transthoracic echocardiography, transesophageal echocardiography (transesophageal three-dimensional echocardiography), left heart contrast echocardiography, right heart contrast echocardiography, intraoperative monitoring during cardiac surgery and interventional treatment, and bedside cardiac ultrasound examination for critically ill patients. Left heart contrast echocardiography: a new technology that has only been applied clinically in recent years, mainly divided into myocardial contrast echocardiography (MCE) and cardiac chamber opacification (LVO). MCE can be used to evaluate myocardial microcirculatory perfusion and accurately determine the extent and severity of coronary microcirculatory disorders and acute and chronic ischemic heart disease lesions;the main clinical indications for LVO: 1、Quantitative evaluation of left ventricular volume and ejection fraction;2、left ventricular apical hypertrophy, thrombus, and ventricular aneurysm; 3、left ventricular myocardial noncompaction;4、differentiation of intracardiac masses. Right heart contrast echocardiography: main indications: 1、patients with suspected patent foramen ovale, such as those with cryptogenic stroke or migraine;2、patients with suspected pulmonary arteriovenous fistula;3、patients with suspected anomalous venous drainage (such as a persistent left superior vena cava draining into the left atrium, etc.);4、patients with decompression sickness, such as divers or pilots, etc.;5、evaluation of the closure effect after patent foramen ovale closure. Transesophageal echocardiography: the probe is inserted directly into the esophagus to examine cardiac structures from behind the heart, effectively avoiding interference with ultrasound waves from the chest wall, pulmonary gas, lung tissue, etc., and producing clearer images. It has clear advantages in detecting left atrial appendage thrombus and patent foramen ovale and facilitates ultrasound monitoring and evaluation during cardiac surgery. Obstetrics and Gynecology Group Composite Photo Introduction to Prenatal Ultrasound Examination Items and Examination Instructions:Ultrasound examination at 11-13+6 weeks of gestation (NT) One of the contents of the ultrasound examination at 11-13+6 weeks of gestation is NT measurement, and NT is one of the screening indicators for fetal Down syndrome. Because of factors such as fetal position and the pregnant woman’s abdominal wall, some fetuses may not have a standard plane available for NT measurement and therefore cannot be included in the combined assessment system for the risk of fetal Down syndrome in early pregnancy. I-level prenatal ultrasound examination I-level prenatal ultrasound examination is mainly intended to determine the number and position of fetuses and measure fetal growth parameters, amniotic fluid, and placenta; it is not an ultrasound examination primarily intended to screen for fetal malformations. Ⅱ-level prenatal ultrasound examination II-level prenatal ultrasound examination is mainly intended to screen for fetal anencephaly, severe encephalocele, severe open spina bifida, severe thoracoabdominal wall defects and visceral evisceration, single-chamber heart, and lethal chondrodysplasia. Because of multiple factors (pregnant woman’s abdominal wall, fetal position, amniotic fluid volume, gestational age, failure of disease progression to result in visualization of abnormal anatomical structures, etc.), the diagnostic accuracy and detection rate for mild cleft lip, cleft palate, congenital heart disease, and skeletal system malformations are relatively low. No abnormality in the fetal four-chamber view cannot exclude all congenital heart disease;issues related to the fetal hands, feet, ears, sex, and reproductive organs are not within the scope of ultrasound examination. Please understand that ultrasound examination has limitations, that changes during fetal development are uncertain, and the importance of follow-up. III-level prenatal ultrasound examination (three-dimensional, four-dimensional) III-level prenatal ultrasound examination is mainly intended to diagnose fetal anencephaly, severe encephalocele, severe open spina bifida, severe thoracoabdominal wall defects and visceral evisceration, single-chamber heart, and lethal chondrodysplasia. Because of multiple factors (pregnant woman’s abdominal wall, fetal position, amniotic fluid volume, gestational age, failure of disease progression to result in visualization of abnormal anatomical structures, etc.), the diagnostic accuracy and detection rate for mild cleft lip, cleft palate, congenital heart disease, and skeletal system malformations are relatively low. No abnormality in the fetal four-chamber view cannot exclude all congenital heart disease;ultrasound displays the long bones of the fetal four limbs but does not include examination of the hands and feet and cannot exclude abnormalities of the fetal hands and feet;issues related to the fetal ears, sex, and reproductive organs are not within the scope of ultrasound examination. Please understand that ultrasound examination has limitations, that changes during fetal development are uncertain, and the importance of follow-up. General Knowledge of Fetal Ultrasound Examination1. Does ultrasound affect the fetus? The ultrasound output energy used in obstetrics is very low, and a particular area is not continuously examined for a long period; the dose of ultrasound examination has no cumulative effect, so even multiple examinations will not cause harm to the fetus or mother. To date, there have been no reports of embryonic death or fetal malformation caused by ultrasound examination.2. How many ultrasound examinations are required during pregnancy? At present, the international medical community generally believes that at least 4—5 ultrasound examinations should be received throughout pregnancy. The purposes and significance of ultrasound examinations at different periods and gestational weeks are not completely the same, and they cannot replace one another. Fetal growth and development is a dynamic process, and different fetal malformations may appear at different developmental stages.3. When should pelvic floor function examination be performed? Women suffering from pelvic floor dysfunction may develop pelvic organ prolapse, urinary incontinence, fecal incontinence, sexual dysfunction, and chronic pelvic pain. Pregnancy and childbirth are the main precipitating factors for PFD. The period from 42 days to 1 year postpartum is the best time to repair the pelvic floor muscles, and the first 6 months postpartum is the golden period for repair. After childbirth, please undergo pelvic floor ultrasound examination as soon as possible, have pelvic floor function assessed, and receive rehabilitation training in a timely manner to avoid missing the optimal period for pelvic floor rehabilitation.
