Orthodontic treatment — braces or clear aligners — straightens teeth and corrects bite problems, and China's public stomatology hospitals now work under government-capped course fees that make headline prices unusually transparent. But orthodontics is the least travel-friendly dental treatment there is: a fixed brace typically stays on for one to two years and needs an in-person adjustment roughly every six to eight weeks, followed by retainers[1]. Treat it as a commitment to a city, not a procedure to fly in for. This page explains who it realistically works for, what it costs, and exactly where the limits of remote follow-up lie.

Who this is realistic for

Orthodontics in China makes sense if at least one of these is true:

  • You live in China — expatriates, international students, long-term workers and their children. This is the standard case, and the follow-up schedule is then no different from having braces anywhere else.
  • You can genuinely return every 6–10 weeks — for example, you work in a neighbouring country or split your time between countries. Be honest about whether this will hold for two years.
  • You are starting a long stay (a degree course, a multi-year posting) that comfortably covers the whole treatment plus the retention phase's early checks.

Who should not start braces in China: tourists and short-stay medical travellers. If you cannot commit to the review schedule, an orthodontist cannot deliver the result safely — and no reputable one will promise otherwise. If your interest is a smaller cosmetic fix, discuss limited local treatment options with providers and read choosing a hospital first; if your teeth need restorative work instead, see crowns and veneers.

Treatment options

All mainstream appliance types are available in China:

  • Fixed metal braces — brackets bonded to each tooth, connected by an archwire that is adjusted over time[1]. Usually the most economical.
  • Ceramic (tooth-coloured) fixed braces — same mechanics, less visible.
  • Lingual braces — fixed to the inner surface of the teeth. In Beijing's public fee schedule, lingual treatment is explicitly classed as "complex", with a higher cap[3].
  • Clear aligners — series of removable transparent trays. Under China's pricing framework, custom-made appliances like aligners and retainers are priced as products by each institution rather than through fixed service items, so aligner totals vary by provider and brand[2][3].

Which is suitable for you is a clinical decision made from X-rays, photos, scans or models and a bite analysis — in Beijing's schedule this diagnostic work-up is its own item (CNY 1,000) that is credited against the course fee if you proceed at the same institution[3].

Process and timeline

  1. Work-up and plan — records, cephalometric analysis, treatment design[3]. Sometimes extractions or mini-screw anchorage (TADs) are part of the plan[3].
  2. Fitting — brackets and wires are bonded, or the first aligner trays are delivered. Teeth are commonly sore for a few days; soft food and ordinary painkillers help[1].
  3. Adjustments — an appointment roughly every 6–8 weeks so the orthodontist can adjust the appliance[1]. Missed visits stretch the timeline.
  4. Appliance removal — typically after 1–2 years for a fixed brace[1]; complex adult cases can take longer.
  5. Retention — retainers (fixed wires and/or removable trays) stop teeth drifting back; your orthodontist tells you how long to wear them[1]. In Beijing, installing a fixed retainer is capped at CNY 600 per arch, and removable retainers are priced as custom products[3].

Throughout treatment you must keep teeth and appliance scrupulously clean and avoid hard, sticky and very sugary foods that break brackets or cause decay[1].

What it costs in China

As of October 2026, orthodontics falls under China's national framework of standardized dental price items; each province sets its own maximum prices for public hospitals[2]. Beijing's schedule — maximum guide prices for public and non-profit institutions, effective 27 June 2026 — is the clearest public benchmark[3][4]:

Item Beijing cap (CNY)
Diagnostic work-up & treatment design (credited against course fee) 1,000
Full course, permanent dentition, Class I — regular / complex 19,000 / 29,900
Full course, permanent dentition, Class II — regular / complex 26,000 / 39,000
Full course, permanent dentition, Class III — regular / complex 26,000 / 38,000
Early treatment, mixed dentition (children) — by class & complexity 9,000–18,000
Limited ("partial") orthodontics, per quadrant per course 8,000
Anchorage mini-screw (TAD), per screw 400
Fixed retainer installation, per arch 600

Three things to understand about these numbers[3][4]:

  • "Complex" includes adults. Beijing's schedule explicitly lists patients over 18, open bite, surgical-orthodontic combined cases and lingual braces among "complex" criteria — so most adult courses fall under the higher caps.
  • The course fee covers the service, not every product. It spans design, fitting, adjustments and removal over the whole course; custom appliances such as clear aligner trays and removable retainers are priced separately by the institution as products[2][3]. Ask for the all-in total in writing.
  • Caps bind Beijing's public and non-profit institutions. Other provinces publish their own levels, private clinics price freely, and relapse re-treatment at the same institution is charged at half the course fee under Beijing's rules[3].

Orthodontics is self-pay for international patients — Chinese basic insurance does not apply to foreign visitors — so check your own cover: see insurance for treatment abroad.

Two price tiers: ordinary vs international/private. The prices above are ordinary-tier (non-VIP) public-hospital prices. In a public hospital's ordinary departments (普通门诊), basic services are billed at government-guided item prices that apply to every patient, foreigners included; 特需 (VIP) and international-department services are priced by the hospital at market rates and capped at a share of its services[6]. Public hospitals sell bulk-procured (集采) drugs and devices at their purchase price, with no markup[7]. As an illustration, China-Japan Friendship Hospital's published schedule lists a chief-physician consultation in its international department at ¥500, against ¥50 for an ordinary attending-physician appointment[8]. You do not have to use the international department: with a passport you can register at the ordinary clinic and pay yourself (mainland insurance does not apply) — see your first hospital visit without a Chinese ID. The international department or a private hospital costs more but usually adds English-speaking staff, appointments and, often, direct billing with commercial insurers. Whichever route you choose, ask for a written quote.

How prices compare abroad

In England, NHS orthodontic treatment is available only where clinically necessary — in practice mostly for under-18s — at the Band 3 charge of £332.10, while adults usually pay for private treatment, whose cost varies with case complexity and appliance type[1][5]. Privately, a multi-year course anywhere is a four-figure commitment in most Western countries. China's capped public-hospital course fees are therefore attractive only if the follow-up logistics genuinely work for you — a cheap course you cannot attend is not cheap. For the general method of comparing quotes across countries, see costs and price comparison.

The follow-up problem — read this before committing

Be explicit with yourself about what cannot be done remotely:

  • Wire adjustments are physical. No orthodontist can tighten, bend or replace an archwire over a video call. Fixed braces without regular adjustment simply stop working — or keep moving teeth the wrong way.
  • Breakages need local hands. A debonded bracket or poking wire needs an in-person fix within days. If it happens at home, you will be paying a local orthodontist who did not plan your case — some will decline to touch another clinician's appliance except to remove it.
  • Aligners tolerate distance only by design. Longer unsupervised stretches are possible when the plan anticipates them (enough trays, clear instructions, photo check-ins), but refinements still require new scans in the chair. Agree the exact visit calendar — dates, not vibes — before paying.
  • Mid-treatment transfers are costly. Records help (get your scans, photos and plan — see medical records and follow-up), but the receiving orthodontist will usually re-plan and re-charge. Beijing's 50%-fee rule for relapse re-treatment applies only at the same institution[3].
  • Retention is part of treatment. Teeth drift back without retainers[1]; broken retainers need replacing promptly, wherever you are.

Choosing a provider

  • Prefer a dedicated orthodontic department (public stomatology hospitals have them) or a licensed clinic where a named orthodontist owns your case end-to-end.
  • Ask for the full written quote: work-up, course fee, appliance/product costs, TADs, retainers, refinement policy, and what happens if treatment runs long.
  • Ask how foreign patients are handled: English-speaking staff, WeChat-based scheduling, and whether the international department coordinates appointments.
  • Ask the transfer question directly: "If I must leave China mid-treatment, what exactly do you hand over, and in what format?"

Browse hospitals offering orthodontics, and use the bilingual inquiry letter tool to ask these questions in Chinese and English before you commit.

Already in braces and moving to China?

The transfer problem also runs in the other direction. If you started treatment at home and are relocating to China:

  • Ask your current orthodontist for a complete record pack: initial records, progress notes, the appliance prescription (bracket system, slot size, wire sequence) or the aligner plan, and recent X-rays.
  • Contact receiving clinics before you fly. Some will continue a foreign plan; others will insist on re-diagnosis and a new course — Beijing's half-price rule for re-treatment applies only to relapse cases at the same institution[3], so a transferred case is normally a new course at full price.
  • Expect a gap of several weeks between arrival and your first adjustment while registration, records review and planning happen; ask your home orthodontist what a safe "holding" interval is for your appliance.
  • Aligner patients should bring all remaining trays plus the digital plan; continuation support varies by provider and brand.

On paying: hospital international departments usually accept bank cards, but daily life in China runs on mobile payment apps — set them up before you travel: see payments with Alipay and WeChat.

Risks and limits

  • Discomfort after fittings and adjustments is normal for a few days[1]; persistent pain, very swollen gums or broken appliances need review[1].
  • Poor cleaning around brackets risks decay and gum problems — daily interdental cleaning and fluoride toothpaste are non-negotiable[1].
  • Timelines are estimates; complex tooth movements, missed visits and breakages all extend treatment.
  • Results relapse without disciplined retainer wear[1].
  • No provider can guarantee a specific cosmetic outcome; be wary of anyone who does. For the general picture, read risks and complications and is it safe? quality standards in China.

Sık sorulan sorular

Can I get braces in China if I only visit once or twice a year?

For fixed braces, no — they need in-person adjustment roughly every 6 to 8 weeks, and a broken bracket needs prompt local care. Some aligner plans tolerate longer gaps, but only if the treating orthodontist explicitly designs and agrees to such a schedule.

How much do braces cost at a Chinese public hospital?

In Beijing, government caps effective June 2026 set a full permanent-dentition course at CNY 19,000–26,000 for regular Class I–III cases and up to CNY 29,900–39,000 for complex cases, which explicitly include adults over 18. Custom appliances such as clear aligners and removable retainers are priced separately by each institution, and private clinics set market prices.

How long does orthodontic treatment take?

Fixed braces commonly stay on for 1 to 2 years, with adjustments every 6 to 8 weeks, and you wear retainers afterwards to stop teeth drifting back. Complex adult cases can take longer.

Can my orthodontist at home finish treatment started in China?

Mid-treatment transfers are possible but messy: systems, materials and treatment philosophies differ, the new orthodontist may restart planning, and you usually pay again. If you may relocate, agree on a transfer plan and full records before treatment starts.

Do adults pay more than children in China?

At Beijing public hospitals, cases in patients over 18 are classed as 'complex', with a higher price cap than regular cases. In many other countries adults also pay more in practice — in England, for example, adults usually pay privately while eligible under-18s are treated on the NHS.
