Introduction to the Anorectal Department

The Anorectal Department of the Second Affiliated Hospital of Heilongjiang University of Chinese Medicine is one of the earliest departments in Heilongjiang Province specializing in the research and treatment of common anorectal diseases,is a key specialty of the Heilongjiang Provincial Administration of Traditional Chinese Medicine,and achieves good therapeutic results for severe hemorrhoids,anal fissures,anal fistulas,perianal abscesses,rectal polyps,rectal mucosal laxity,constipation,colitis,and other conditions.

Our department has extensive experience,strong capabilities,considerable scientific research strength,and abundant expert resources,and currently has 15 medical and nursing staff members,2 chief physicians,2 attending physicians,and 1 resident physician,including 2 supervisors of master’s students and 5 staff members with master’s degrees.The department is equipped with advanced diagnostic and treatment equipment and therapeutic instruments.

It currently has 1 Longjiang Famous Physician and 2 university-designated famous TCM physicians,has completed 1 Heilongjiang Provincial Natural Science Foundation project,co-authored 3 national-planning textbooks,published dozens of academic papers,is a key specialty of the Heilongjiang Provincial Administration of Traditional Chinese Medicine,and has received honors including the Third Prize for Scientific and Technological Achievements on multiple occasions,the Second Prize for New Technology from the Heilongjiang Administration of Traditional Chinese Medicine,and the National Invention Award.It is a teaching and medical-talent training base for undergraduates of Heilongjiang University of Chinese Medicine,and has trained several outstanding medical professionals.

Departmental Specialty Treatments

1,Minimally invasive,painless therapy:rph procedure,TST procedure,and Xiaozhiling injection therapy are used to treat rectal mucosal laxity,mixed hemorrhoidal prolapse,and other conditions causing hematochezia,prolapse,difficulty defecating,a sensation of anal heaviness,and a feeling of incomplete evacuation,with significant therapeutic efficacy,a low recurrence rate,no pain,and rapid healing.The innovative use of retrograde ligation and excision of mixed hemorrhoids makes the incision size controllable,prevents anal stenosis caused by excessive excision,and applies distinctive TCM treatment during surgery to improve the surgical outcome.

2,Painless treatment of anorectal diseases throughout the entire course.Before surgery,patients with severe pain are not given enema preparation,reducing their preoperative suffering,and no medication is administered by intramuscular injection before surgery,alleviating anxiety caused by pain.During surgery,a self-developed long-acting analgesic is used for postoperative analgesia after anal surgery,solving the difficult problem of pain after anal surgery.After surgery,multiple pain-relief measures are adopted,including oral medication,an analgesia pump,TCM herbal sitz baths,and analgesic suppositories,so that patients are essentially pain-free after surgery.

3,For patients with relatively severe hemorrhoids or fistulas,anal surgery is performed from a "reconstructive" perspective.It features no anal deformity and no recurrence.

4,Oral TCM methods for replenishing qi,moistening the intestines,and facilitating defecation are used,together with acupuncture,acupoint injection,and other treatment methods,to effectively treat functional constipation in older adults and outlet obstructive constipation caused by internal rectal mucosal prolapse.For patients with constipation and colitis,comprehensive symptomatic treatment is used.In clinical practice,we found that patients with constipation have sticky stools,indicating a damp-heat type;therefore,heat-clearing and dampness-draining medicines,such as gentian and gardenia,are innovatively used in the constipation-relieving TCM prescriptions,and good therapeutic results have been achieved5,Using incision and seton placement and open-drainage therapy to treat perianal abscesses.

Perianal abscess has an acute onset and severe condition,and if it is not treated promptly,it may even be life-threatening.At present,many hospitals perform staged surgery for treatment.The two operations not only increase patients’ suffering and their financial burden,but also seriously affect their physical and mental health.Based on summarizing the TCM seton method for treating anal fistula,we perform one-stage incision and seton placement for perianal abscess,which can cure the condition with a single operation,avoids the pain of a second operation for an anal fistula,shortens the hospital stay,and reduces hospitalization costs,reaching an advanced level within the province.
