III: Departmental Features and Technical Advantages:
Since its establishment, the Gynecology Department of the East Hospital has been committed to treating women's health conditions, with minimally invasive surgery as its specialty. In 2004, it took the lead in the island city in performing a series of novel vaginal surgeries, such as vaginal hysterectomy, subtotal hysterectomy, transvaginal myomectomy, and transvaginal ovarian cystectomy, among others. These procedures offer the advantages of no abdominal incision or scar and less trauma. It also took the lead in Qingdao in performing laparoscopic minimally invasive surgical treatment for various benign and malignant gynecologic tumors, as well as various hysteroscopic surgeries, and was approved as a Qingdao Specialty Department for Gynecologic Minimally Invasive Surgery. At the beginning of 2017, it took the lead in Qingdao in performing less traumatic transumbilical single-port laparoscopic gynecologic surgery and transvaginal single-port laparoscopic surgery (VNOTES), achieving a micro-traumatic, nearly noninvasive effect. In December 2018, it took the lead in Qingdao in performing uterine-manipulator-free laparoscopic radical surgery for cervical cancer and comprehensive staging surgery for endometrial cancer without a uterine manipulator; while being minimally invasive, these procedures are more consistent with the no-touch tumor principle. In 2019, it took the lead in Qingdao in performing gynecologic malignant tumor surgery with a fourth-generation da Vinci robot. Compared with traditional laparoscopic surgery, it is more flexible, precise, and stable, achieving good clinical results and enabling the development of gynecologic surgery from minimally invasive to nearly noninvasive procedures, and from nearly noninvasive to intelligent surgery. In recent years, the department has joined with the General Surgery, Urology, Hepatobiliary Surgery, Anesthesiology, ICU, and other sister departments to establish a gynecologic oncology MDT diagnosis and treatment platform. It has accumulated extensive clinical experience in fertility-preserving surgery for early gynecologic malignant tumors, comprehensive staging surgery for early malignant tumors, radical surgery for advanced malignant tumors, postoperative adjuvant radiotherapy and chemotherapy, and targeted and immunotherapy, achieving good clinical outcomes. This has significantly improved patients' tumor-free survival rate, survival rate, and quality of life. At present, the department's minimally invasive techniques, surgery and comprehensive treatment of gynecologic malignant tumors, whole-course management, and surgical treatment of female pelvic organ prolapse and other conditions have all reached leading levels within the province and nationwide.
After years of dedicated cultivation and development, the department has established seven stable and mature core specialty areas: minimally invasive gynecology, gynecologic oncology, cervical lesions, pelvic floor dysfunction disorders, reproductive endocrinology, intrauterine diseases, and endometriosis. Its specialized strengths are prominent, and its brand recognition is high. Subspecialty introduction:
1. Gynecologic Oncology Subspecialty: Our department is the institution chairing the Ovarian Cancer Multidisciplinary Diagnosis and Treatment Branch of the Shandong Society for Translational Medicine. It is the earliest hospital in Qingdao to perform laparoscopic radical surgery for gynecologic malignancies, and specializes in surgical treatment, adjuvant radiotherapy and chemotherapy, and targeted maintenance therapy for all types of gynecologic malignancies. We now comprehensively perform radical or comprehensive staging surgery for early-stage endometrial cancer, early-stage cervical cancer, and ovarian cancer via open abdominal surgery, laparoscopy, single-port laparoscopy, and the da Vinci robot; among these, para-aortic lymph node dissection has reached the level of the renal vessels. We perform surgeries for vulvar and vaginal malignancies, including radical vulvectomy plus laparoscopic inguinal lymph node dissection for vulvar cancer, and laparoscopic total vaginectomy plus pelvic and abdominal lymph node dissection for vaginal cancer. For advanced ovarian or fallopian tube cancer, we perform carpet-like cytoreductive surgery; together with the Departments of General Surgery, Urology, Hepatobiliary Surgery, Vascular Surgery, Anesthesiology, and others, we have established an MDT multidisciplinary collaborative diagnosis and treatment platform and perform total, anterior, middle, and posterior pelvic exenteration, as well as resection and reconstruction of metastatic organs. We have introduced genetic testing technology for gynecologic malignancies and carry out targeted and immunotherapies, providing patients with the latest and more precise treatment. We emphasize standardized, individualized, and human-centered long-term comprehensive management of cancer treatment, integrating surgery, chemotherapy, radiotherapy, and targeted maintenance therapy to reduce tumor recurrence and continuously improve patients’ overall survival and quality of life.
2. Gynecologic Endoscopy and Robotic Subspecialty: Our department is a specialized gynecologic minimally invasive unit in Qingdao, a standardized gynecologic endoscopy training base in Shandong Province, and a member institution of the Shandong Provincial Gynecologic Endoscopy Specialty Alliance. Gynecologic minimally invasive surgery is our department’s signature service. We currently perform a full range of minimally invasive procedures under multi-port laparoscopy for various benign and malignant gynecologic tumors. At present, 95% of surgeries can be completed using minimally invasive techniques; the technology is mature and advanced and has received unanimous recognition from the people of Qingdao. In early 2017, we took the lead in Qingdao in performing the less traumatic transumbilical single-port laparoscopic surgery and transvaginal single-port laparoscopic surgery (VNOTES surgery). Compared with traditional laparoscopic surgery, these procedures cause less trauma, enable faster recovery, involve smaller and more concealed incisions, better provide cosmetic benefits and protect patient privacy, and more closely conform to the concepts of minimally invasive treatment and enhanced recovery. They have received unanimous acclaim from the majority of patients, achieving the development from minimally invasive to nearly non-invasive surgery. This technology is currently at an advanced level within Shandong Province and across China. Over the past nearly 7 years, using da Vinci robotic surgery as a new breakthrough point, we have continuously advanced the development and innovation of intelligent gynecologic surgery and enhanced the department’s new competitiveness. Our department has now completed multiple robotic surgeries for benign and malignant gynecologic tumors, with good results. In the future, we will continue to vigorously perform various gynecologic surgeries with robotic assistance, bringing gynecologic surgery from the era of endoscopy into the era of robotic surgery and achieving the development of gynecologic surgery from minimally invasive treatment toward intelligent treatment.
