It comprises two inpatient wards, at the Main Campus and East Campus, as well as the Clinical Center of the Ministry of Health Key Laboratory of Cell Transplantation, a choledochoscope room, and a duodenoscope room. The department currently has 79 beds, with 88 beds in operation, and is equipped with advanced surgical and therapeutic equipment, including an electrosurgical workstation, intraoperative color Doppler ultrasound, a radiofrequency therapy system, a microwave therapy system, high-definition laparoscopes, pancreatoduodenoscopes, equipment for Oddi’s sphincterotomy and stone extraction, and electronic choledochoscopes. The department is renowned for its strong capacity to manage difficult and severe hepatobiliary diseases.
Distinctive Features and Technical Advantages
The discipline comprehensively covers the medical, teaching, and research work related to all surgical diseases of the liver, biliary tract, pancreas, spleen, and duodenum. This includes totally minimally invasive surgery for complex biliary stone disease using multiple endoscopes in combination; surgical and comprehensive treatment of liver tumors, pancreatic tumors, and hilar cholangiocarcinoma; and extensive experience and mature techniques in the diagnosis and surgical treatment of severe acute pancreatitis, portal hypertension, and other diseases. In recent years, while continuing to inherit and innovate, the department has focused on developing two major directions—precision surgery and minimally invasive surgery—on the basis of its original key technical strengths. It has achieved multiple breakthroughs and advances in both theory and technology, established mature subspecialties, and introduced a number of new technologies and projects, all of which encompass research hotspots and technical challenges in this field both in China and internationally.
Discipline Development
In the area of precision surgery, the department has developed digital liver surgery techniques and 3D virtual imaging technology, and performs precise segmentectomy, anatomical hepatectomy, standard hemihepatectomy, and extended hemihepatectomy. Among these, precise anatomical liver segmentectomy and combined liver segmentectomy using indigo carmine staining, as well as complex liver tumor resection assisted by 3D virtual imaging, have reached a domestically leading level. Radical resection of hilar cholangiocarcinoma with perihilar resection, and radical resection of hilar cholangiocarcinoma combined with hepatectomy and vascular reconstruction, have likewise reached a domestically leading level. In pancreatic and splenic surgery, the department performs pancreatoduodenectomy with portal vein–superior mesenteric vein resection and reconstruction, middle segment pancreatectomy, pylorus-preserving pancreatoduodenectomy, extended pancreatoduodenectomy, and comprehensive treatment of severe acute pancreatitis centered on surgical techniques. Combined treatment techniques, including interventional therapy, radiofrequency ablation, microwave therapy, particle implantation radiotherapy, guided chemotherapy, and biological targeted therapy, are also at an advanced level in China.
In the minimally invasive surgery subspecialty, the department’s current minimally invasive surgery rate has reached 75%-80%. Its overall technical capabilities rank among the leading hepatobiliary surgery departments in Qingdao and Shandong Province, and it has developed into a regional specialty center. The department’s completed procedures include totally laparoscopic precise liver segmentectomy/lobectomy, totally laparoscopic pancreatoduodenectomy, totally laparoscopic distal pancreatectomy with splenectomy, totally laparoscopic middle segment pancreatectomy, totally laparoscopic splenectomy for massive splenomegaly, precise splenectomy with level-II splenic pedicle division, precise partial splenectomy and hemisplenectomy, and totally laparoscopic splenectomy and devascularization, all of which are at a domestically leading level.
In the endoscopic minimally invasive surgery subspecialty, the department is equipped with laparoscopes, choledochoscopes, duodenoscopes, and endoscopic ultrasound, and was among the earliest in the province to perform minimally invasive surgery for biliary stone disease using the “four-in-one endoscopic” technique. Techniques for treating complex intrahepatic and extrahepatic bile duct stones with combined hepatectomy, and totally minimally invasive treatment of intrahepatic and extrahepatic bile duct stones using multiple endoscopes, include single-port laparoscopic biliary surgery; totally laparoscopic common bile duct exploration and stone extraction combined with choledochoscopy, including T-tube drainage or primary closure of the common bile duct; ERCP via duodenoscopy for the treatment of intrahepatic and extrahepatic bile duct stones; percutaneous transhepatic single-port choledochoscopy (PTCS) for the treatment of intrahepatic and extrahepatic bile duct stones; and radiation-free three-endoscope combined treatment of biliary stone disease. These techniques have made the treatment of biliary stone disease comprehensively minimally invasive, and the minimally invasive surgery rate for biliary stone disease has reached over 90%. The department also performs endoscopic resection of duodenal papillary tumors, laparoscopic combined with endoscopic ultrasound treatment of multiple intrahepatic cysts, endoscopic ultrasound-guided transgastric catheter drainage of pancreatic pseudocysts, and transgastric catheter drainage of intrahepatic bile ducts, among other techniques, reaching an advanced level in China. In July 2018, the Qingdao ERCP Technology Training Center was officially inaugurated at the Municipal Hospital Group, with the aim of promoting the standardized dissemination of ERCP technology in the Shandong Peninsula region and benefiting more people in the peninsula.
The department took the lead in implementing joint physician-nurse ward rounds and establishing an enhanced recovery after surgery ward. Through a series of measures implemented before surgery, during the perioperative period, and after surgery, patients can achieve rapid recovery while the standard of medical care is improved.
