Cataract surgery — replacing the eye's clouded natural lens with an artificial intraocular lens (IOL) — is one of the most routine operations in Chinese hospitals, and since the 2023 national procurement round the lenses themselves cost on average about 60% less in public hospitals[1]. For an international patient the medicine is straightforward; the planning is not: both eyes are usually done as separate procedures, follow-up visits anchor your schedule, and the total price depends heavily on which lens you choose. Plan one trip of one to three weeks.

Why consider China for cataract surgery

Cataract is overwhelmingly a disease of aging — Chinese health-insurance authorities note an incidence above 80% among people over 60[2] — so Chinese ophthalmic departments perform these operations at enormous scale, typically by phacoemulsification (ultrasound breaking up the lens through a tiny incision). Two things changed the economics in recent years:

  • National IOL procurement. In November 2023, China's fourth national high-value consumables tender covered the full range of intraocular lenses — monofocal, bifocal, trifocal and extended depth-of-focus (EDOF) — cutting IOL prices by about 60% on average, with projected savings of ¥3.9 billion per year; patients nationwide got the new prices from May–June 2024[1].
  • Concrete lens prices. Examples from the tender: an imported EDOF lens fell from ¥11,000 to about ¥3,500, a new-generation imported monofocal from ¥10,000 to about ¥1,100[1], and a reported patient case saw a lens drop from ¥11,066 to ¥6,990[3]. The most-used domestic monofocal lenses now cost in the hundreds of yuan.

For comparison, cataract surgery in the US without insurance starts around $3,000 per eye, and premium-lens packages can start near $6,000 per eye[7]. Note the procurement prices above are the lens component in public hospitals; surgeon, facility, medication and exam fees are billed on top, and international patients pay out of pocket at self-pay rates. See hospitals performing cataract surgery.

Choosing a lens: where most of the money goes

IOL type What it does Cost logic
Monofocal Sharp focus at one distance; reading glasses usually still needed Cheapest, most predictable — the standard worldwide
Toric Corrects astigmatism as well Moderate premium
Multifocal / trifocal Several focal distances; less dependence on glasses Significant premium (tender prices roughly ¥9,000–14,500 for trifocals[1]); not every eye tolerates halos/glare
EDOF One extended focus zone for intermediate + distance Between monofocal and trifocal

Premium lenses are where quotes balloon — in any country[7]. A good surgeon will justify a premium lens from your biometry, your astigmatism and your lifestyle, not from the price list. It is entirely reasonable to ask: "Would you implant this lens in your own parent's eye with these measurements?"

Candidacy and the pre-op exam

The workup — slit-lamp exam, dilated fundus exam, biometry (measuring the eye to calculate lens power), corneal assessment, sometimes OCT — determines whether surgery will actually help and which lens power you need. It can also postpone or redirect you: uncontrolled glaucoma or diabetes-related retinal disease, active eye inflammation, or a cataract that is not yet the main cause of poor vision are all reasons a careful surgeon may advise against operating now. Prior refractive surgery (LASIK/SMILE) makes lens-power calculation harder — bring your old records; see our refractive surgery article for why those parameters matter decades later.

If you take blood thinners or have significant heart/lung conditions, say so in your first inquiry — cataract surgery under local anesthesia is low-stress, but the hospital may want recent reports or an internal-medicine review.

Process and trip length

A realistic plan (example):

  • Days 1–2 — consultation and biometry. Half a day to a full day. Lens choice and price are fixed here; get the quote itemized (surgery fee, lens, drugs, exams).
  • Day 3 — surgery on the first eye. Outpatient; the operation itself takes minutes to tens of minutes, and you leave the same day with a shield and drops.
  • Day 4 — next-day review. Standard practice: most ophthalmologists want to see patients the first day after surgery and again about a week later[6].
  • Day 10 or so — one-week review. If only one eye is planned, many travelers fly after this check.
  • Second eye: usually a separate procedure days to weeks after the first — this is what stretches a both-eyes trip to roughly two to three weeks. Ask your surgeon what interval they use before you book anything.

Vision typically improves within days, but the final glasses prescription is usually written weeks after surgery — plan to get the definitive spectacles at home, or see eyeglasses & optometry in China if your schedule allows a late-trip fitting.

After surgery: flying, water, activity

  • Flying: for most patients, flying after routine cataract surgery is safe — the operation does not usually create pressure-related concerns in the cabin[4]. The real constraints are the day-1 and week-1 reviews[6]. Critical exception: if air or gas was placed in the eye (rare in cataract surgery, more common in retinal procedures), a gas bubble can expand dangerously at altitude and threaten sight — do not fly until your doctor confirms it is gone[4].
  • Water: wait about two weeks before swimming — the common baseline to minimize infection risk — and note that depending on the surgery, swimming in any water may be restricted for up to 4 weeks[5]. You can shower the day after, but keep the water stream away from the eye[5].
  • Activity: light activities like biking, running, tennis or golf generally resume after 7–10 days[5]; avoid heavy lifting and eye rubbing early on; wear the shield at night as instructed.
  • Red flags: vision loss, eye pain, flashing lights or a dark curtain over vision need immediate contact with a doctor[4] — know before you travel home where you would go.

Follow-up at home and the long tail

Routine checks continue around one month after surgery, and a final refraction sets your new glasses. Months or years later, some patients develop posterior capsule opacification ("secondary cataract"), treated in minutes with a YAG laser — a standard service anywhere, but one more reason to carry complete records. Before leaving China, obtain:

  • the operation note and the IOL sticker (model, power, serial number) for each eye — future doctors need these exact identifiers;
  • biometry printouts and the latest refraction;
  • the drop schedule in English, and written red-flag instructions.

Our medical records guide explains formats that clinics abroad accept. Balanced view: if you have significant retinal disease, advanced glaucoma, or you cannot arrange competent follow-up at home, a two-flight treatment plan adds real risk — discuss with a local ophthalmologist first, and read risks and complications.

Who should not travel for this: patients with one-eye-only vision, complex ocular comorbidities, or anyone who cannot stay for the follow-up schedule. Cataract surgery is rarely urgent; a postponed operation is usually a safe operation.

Costs: what a quote should contain

There is no single national package price, so insist on an itemized quote: surgeon/facility fee, the lens (by exact model), pre-op exams, medications and review visits. The procurement reform means the lens line on your bill should look dramatically lower than pre-2024 price lists[1] — if a quote for a standard monofocal package looks like it predates the reform, ask why. Chinese public insurance covers much of this surgery for locals[2] but does not apply to international visitors; some foreign private policies reimburse medically necessary cataract surgery abroad — check before you travel via our insurance guide, and compare totals (including flights and 2–3 weeks of accommodation) with home-country self-pay prices[7] using the cost comparison guide.

Two price tiers: ordinary vs international/private. The prices above are ordinary-tier (non-VIP) public-hospital prices. In a public hospital's ordinary departments (普通门诊), basic services are billed at government-guided item prices that apply to every patient, foreigners included; 特需 (VIP) and international-department services are priced by the hospital at market rates and capped at a share of its services[8]. Public hospitals sell bulk-procured (集采) drugs and devices at their purchase price, with no markup[9]. As an illustration, China-Japan Friendship Hospital's published schedule lists a chief-physician consultation in its international department at ¥500, against ¥50 for an ordinary attending-physician appointment[10]. You do not have to use the international department: with a passport you can register at the ordinary clinic and pay yourself (mainland insurance does not apply) — see your first hospital visit without a Chinese ID. The international department or a private hospital costs more but usually adds English-speaking staff, appointments and, often, direct billing with commercial insurers. Whichever route you choose, ask for a written quote.

คำถามที่พบบ่อย

How long should I stay in China for cataract surgery?

Plan 1–3 weeks. Surgery itself is an outpatient procedure, but surgeons want to see you the day after and again about a week later, and the second eye is usually operated on days to weeks after the first.

Can both eyes be done in one trip?

Often yes, as two separate procedures spaced days to weeks apart — that is what pushes trips toward two to three weeks. Confirm the interval your surgeon uses before booking flights.

Is it safe to fly home after cataract surgery?

For most patients flying after routine cataract surgery is safe; the real constraint is the follow-up schedule. The exception is surgery where gas or air was placed in the eye — then flying is dangerous until your doctor clears you.

How much cheaper are lenses after China's national procurement?

In the 2023 national tender, intraocular lens prices fell about 60% on average: a widely used domestic monofocal lens fell to roughly ¥800, one imported EDOF lens from ¥11,000 to about ¥3,500, and trifocal lenses to roughly ¥9,000–14,500. Hospital surgical and exam fees are separate.

Which lens should I choose?

Monofocal lenses are the standard, cheapest and most predictable; multifocal/trifocal and EDOF lenses reduce dependence on glasses but cost more and don't suit every eye. The choice depends on your measurements and habits — ask the surgeon to justify any premium lens for your case.

What records do I need for my eye doctor at home?

An operation note with the exact IOL model, power and serial sticker, biometry printouts, and the post-op refraction. Any clinic at home can then manage your remaining checks and prescribe final glasses.
