What are the responsibilities of an internal medicine physician? Internists focus on the prevention, diagnosis, and treatment of diseases in adults. Unlike a family physician or general practitioner, an internist does not handle obstetric and gynecologic, pediatric, or surgical problems.

Internists often serve as detectives or consultants for other departments, providing expert management advice when faced with difficult or complex diagnostic problems.

In China, internists usually specialize in one field after graduation, such as cardiovascular medicine, respiratory medicine, or gastroenterology. In countries such as the United States and the United Kingdom, general internists can generally diagnose and initially treat a broader range of common problems, including cardiovascular diseases (coronary heart disease, arrhythmia, hypertension, and others), pulmonary diseases (asthma, chronic obstructive pulmonary disease, pneumonia, and others), gastrointestinal diseases (gastritis, hepatitis, diverticulitis, and others), blood diseases (anemia, thrombocytopenia), endocrine diseases (diabetes, thyroid problems), rheumatic diseases (arthritis, gout, vasculitis, systemic lupus erythematosus, and others), common neurological problems, abnormal kidney function, and various infectious diseases.

In addition, general internists are responsible for routine physical examinations, colds and fever, food poisoning, vaccinations, and a range of preventive-medicine and routine outpatient tasks. It is precisely this broad exposure to diseases that makes general internists better suited to the role of “detective,” because most patients are uncertain at their initial consultation which specialty should address their problem.

Another important responsibility of general internists is to diagnose and manage diseases in hospitalized internal medicine patients. Although a small number of family physicians or general practitioners are willing to care for hospitalized patients throughout their stay, most are responsible only for outpatient treatment. This responsibility of general internists ensures a degree of continuity for patients with chronic diseases in disease management and recovery.
