Flagship specialty
Ocular Trauma Department
Our hospital established the specialty of ocular trauma in 1963. The late renowned ophthalmology experts Wei Jingwen and Director Wu Guifang made creative contributions to this specialty. On this basis, the Tianjin Eye Disease Emergency Center was established in 1995, providing effective treatment for a large number of patients with trauma and external eye diseases. The Ocular Trauma Department currently has 10 physicians with senior professional titles, including 6 doctors with doctoral degrees, 1 with a master's degree, 1 master's student supervisor, and 4 re-employed physicians with senior professional titles. Through the efforts of several generations, this specialty has reached a high level not only in the first-stage treatment of ocular trauma and second-stage reconstruction of the eyeball, but has also accumulated extensive clinical experience in complex cataract surgery, refractory glaucoma, and vitreoretinal diseases. The department head, Director Jiang Hao, undertook further training at the Peking University Eye Center in 2004, studying fundus disease treatment and vitreoretinal surgical techniques under Professor Ma Zhizhong, a nationally renowned expert in ocular trauma and vitreoretinal surgery. It has now accumulated extensive experience in vitrectomy treatment of severe ocular trauma, reaching a particularly high level in the treatment of infectious endophthalmitis, post-traumatic eyes with no light perception, and perforating eye injuries. Department specialties The ocular trauma team led by Director Jiang Hao, with great enthusiasm for work and outstanding professional and technical capabilities, strives to provide the general public with convenient, high-quality, and safe medical services. The Ocular Trauma Department is actively performing various complex routine surgeries, such as lens subluxation, complete lens dislocation, intraocular lens dislocation, small-pupil cataract, refractory glaucoma, rhegmatogenous retinal detachment, proliferative diabetic retinopathy, and so on. Department programs For cases of lens subluxation, phacoemulsification with capsular tension ring implantation combined with intraocular lens implantation, capsular tension ring suturing and fixation in the ciliary sulcus combined with IOL implantation, IOL suspension surgery, and other procedures can be performed with the assistance of a capsular bag retractor, enabling patients to regain good vision. For small-pupil cataracts, cataract phacoemulsification combined with intraocular lens implantation can be performed with the assistance of an iris retractor. For glaucoma patients, angle separation surgery can be performed with gonioscope assistance, as can trabeculectomy, endoscopic cyclophotocoagulation, cataract phacoemulsification combined with trabeculectomy, cataract phacoemulsification combined with endoscopic cyclophotocoagulation, and so on. Intraocular injection of anti-VEGF drugs combined with filtering surgery is used to treat neovascular glaucoma. For cases of iridodialysis, closed iridodialysis repair is performed, enabling patients to regain a good appearance through minimally invasive surgery. For complex ciliary body detachment cases, the angle can be examined under direct visualization during surgery; depending on the specific situation, the ciliary body can be sutured by various methods, including the ab interno approach, ab externo approach, combined ab interno and ab externo approaches, and vitrectomy, achieving a high surgical success rate. 23G and 25G vitrectomy are also used for the surgical treatment of complex retinal detachment, diabetic retinopathy, macular holes, epiretinal membranes, and vitreous hemorrhage caused by various conditions. Artificial vitreous balloon implantation has been performed since 2017. The foldable artificial vitreous changes the traditional retinal support mode, providing 360-degree arcuate solid support for the retina and persistent tamponade for holes in any part of the retina, restoring the natural vitreous's function of maintaining and supporting the shape of the eyeball, avoiding adverse reactions caused by direct contact between silicone oil and intraocular tissues, limiting silicone oil emulsification and displacement, not affecting ciliary body function, and imposing no special requirements on patient positioning. Foldable artificial vitreous implantation greatly increases postoperative comfort for elderly patients and patients unable to tolerate the prone position. Silicone oil can be withdrawn through the drainage valve, or physiological saline and silicone oil can be injected, thereby adjusting intraocular pressure. The foldable artificial vitreous can remain in the eye long term, solving problems such as vitreous substitutes being unable to remain in the eye long term, being unable to provide long-term retinal tamponade, or requiring repeated surgery, and avoiding enucleation and implantation of an artificial orbital implant. Department equipment This department has advanced medical equipment, such as an integrated cataract phacoemulsification and vitrectomy machine, a fiber-free vitrectomy surgical system, an intraocular endoscopic laser surgical system, and so on. At present, the trauma department performs nearly 2000 surgeries annually and serves ophthalmic patients throughout the Beijing-Tianjin-Hebei region. Featured services The trauma department provides patients in Tianjin and surrounding areas with 24-hour emergency surgical services for all types of ocular trauma, such as open-globe ocular trauma repair, intraocular foreign body removal, canalicular anastomosis, and so on, and performs second-stage traumatic cataract surgery, intraocular lens implantation, vitrectomy, anti-glaucoma surgery, ciliary body suturing and fixation, amniotic membrane transplantation, and other operations. Especially for emergency cases such as intraocular foreign bodies and endophthalmitis, experienced physicians in the trauma department can perform emergency vitrectomy for patients; for severely traumatized eyes complicated by retinal detachment and choroidal detachment, including some eyes with no light perception and perforating eye injuries, good results can be achieved through vitrectomy. Research and innovation In the past 5 years, the department has undertaken 2 National Natural Science Foundation projects, 1 Tianjin Natural Science Foundation project, and 3 Tianjin Municipal Health Commission science and technology projects. To date, it has published more than 20 SCI articles as first author and more than 10 articles in Chinese core journals. Each year, it admits dozens of visiting physicians and graduate students for study and has considerable influence in Tianjin, Beijing, and surrounding areas.
